About The Position

Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Fredericksburg and surrounding communities. We have patient opportunities in Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and nearby areas. You do NOT have to cover the entire territory. Choose the areas that work best for you and build your caseload around your availability. Home health PT lets you see what progress looks like in a patient's real life. You're helping someone safely climb their own stairs, get out of their favorite chair, improve their balance, regain strength, and move through their home with greater confidence. Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers on mobility and home exercise programs
• Monitor progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with nurses, physicians, therapists, and other team members
• Complete accurate and timely documentation using WellSky
