About The Position
Central Gardens Post Acute is seeking dedicated Physical Therapists (PTs) to join their premier, in-house therapy department. The role offers professional growth and personal balance, with a focus on helping residents reclaim their independence. The facility provides a supportive environment for both seasoned clinicians and new graduates, offering tools and culture for success. The position is PRN (as needed) and located in the Fillmore District of San Francisco, CA.
Requirements
- Current, valid California Physical Therapist (PT) License.
- A compassionate, resident-centered approach with a desire to work in a collaborative, team-based environment.

Responsibilities
- Evaluate and design evidence-based treatment plans for both short-term rehabilitation and long-term care residents.
- Lead high-quality sessions focused on restoring mobility, building strength, and fostering independence.
- Partner with the interdisciplinary team to enhance resident quality of life and participate in meaningful care planning.
- Provide guidance to PTAs and educate nursing staff on safe mobility and restorative care techniques.
