About The Position

The Physical Therapist is responsible for evaluating patients, developing treatment plans, and providing therapeutic interventions. This role involves assessing patient status, setting functional goals, and modifying treatment as needed. The position also requires accurate and timely documentation, including clinical notes and billing information, as well as effective communication with inter-disciplinary staff, patients, and their families. Productivity standards are set by the department annually.

Requirements

  • Must have a thorough knowledge of treatment techniques and apply them appropriately.
  • Must be able to perform thorough and accurate evaluations.
  • Must be able to determine functional and measurable short and long term goals.
  • Must be able to design an appropriate treatment program.
  • Must be able to consider discharge options at time of initial evaluation.
  • Must be able to modify treatment as necessary.
  • Must be able to update goals and re-evaluate discharge plans.
  • Must include patient and family education.
  • Must communicate and interact with appropriate team members.
  • Must meet productivity requirements and goals.
  • Must ensure clinical documentation is timely, objective, and complete.
  • Must complete administrative documentation, including billing and outcome measures, accurately and timely.
  • Must interact with members of the inter-disciplinary staff pertaining to the welfare and management of assigned patients in a professional manner.
  • Must communicate with patients and family in a respectful, professional, and compassionate manner.

Responsibilities

  • Obtains pertinent medical and social history.
  • Performs thorough and accurate evaluations using techniques to objectively measure physical and functional status, including those specific to specialty area.
  • Determines functional and measurable short and long term goals and designs an appropriate treatment program.
  • Considers discharge options at time of initial evaluation.
  • Performs treatment as outlined in the initial evaluation.
  • Modifies treatment as necessary, updates goals and re-evaluates discharge plans throughout the course of treatment.
  • Demonstrates thorough knowledge of treatment techniques and applies appropriately.
  • Includes patient and family education in treatment.
  • Communicates and interacts with appropriate team members.
  • Meets productivity requirements and goals.
