About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Santa Clarita and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule, taking as much time off as you want. With strong per-visit rates, you determine how much you earn by the number of patients you decide to treat. Our team provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. We offer one-on-one treatment where you observe, record, and report the patient's response to treatment and changes to the patient's condition, and you determine the plan of care. Our Electronic Medical Record system is extremely intuitive, making documentation a breeze, with many clinicians completing documentation during the visit. Requirements

• Current Physical Therapist license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Ability and enthusiasm in working with all ages.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using an intuitive Electronic Medical Record system.
