About The Position

Within an Interdisciplinary Team (IDT) setting, the Physical Therapist is responsible for promoting and maintaining the physical mobility and well-being for Participants both in the Center and at home if needed. This role involves conducting assessments, treatments, and providing teaching and counseling to Participants and family members. The Physical Therapist will also coordinate and mentor restorative aides, center health aides, and home care aides to support restorative care plans.

Requirements

  • Graduate of an accredited Physical Therapy Program.
  • Current/valid Commonwealth of Pennsylvania Physical Therapy License.
  • Valid Driver's license and automobile insurance.
  • Must have a vehicle for travel.
  • Minimum of (1) year diversified clinical experience with the geriatric population.
  • Experience working with and relating to the geriatric population and their family members.

Responsibilities

  • Promoting and maintaining the physical mobility and well-being for Participants both in the Center and at home if needed.
  • Conducting assessments, treatments, and providing teaching and counseling to Participants and family members.
  • Coordinating and mentoring restorative aides, center health aides, and home care aides to support restorative care plans.
  • Assessing pre-enrollment candidates for appropriateness of community-based living within a PACE program, identifying necessary interventions for gait, balance, coordination, transfer, and functional mobility deficits.
  • Addressing skilled therapy needs including gait training, device utilization, strengthening, endurance, balance training, pain management, and participant/caregiving training and education.
  • Proactively identifying physiological/psychological changes in Participants through ongoing assessments and IDT collaboration.
  • Implementing interventions and strategies to address changes for highest functioning possible.
  • Monitoring and assessing current enrollees to ensure the highest level of function and independence in community living settings.
  • Identifying skilled or restorative therapy needs through evaluations and assessments.
  • Instructing family caregivers and caregivers from LP and home health agencies for therapeutic continuity, ensuring follow-up and monitoring.
  • Demonstrating strong clinical reasoning, deduction, and problem-solving to promote participant self-actualization, community living, and aging in place.
  • Coordinating with the interdisciplinary team to develop a comprehensive care plan with a focus on restoring function and independence.
  • Ensuring detailed and thorough documentation, including objective assessment information, plan of care, goals, and progress notes.
  • Staying current on new assessment and treatment protocols for the geriatric population.
  • Utilizing clinical research skills to investigate and apply new treatment approaches.
  • Adjusting communication style effectively for clear and positive exchange of information.
  • Communicating with caregivers and participants regularly to build rapport and explain functional performance.
  • Assessing and treating various geriatric functional areas including gait, bed mobility, transfers, mobility devices, wound care, pain management, vestibular and coordination, sensory deficits, balance, fall prevention, and cognitive limitations.
  • Presenting recommendations of needed interventions, treatments, and equipment to the IDT based on objective assessment findings.
  • Providing services promptly, exercising tact, patience, and courtesy, and conducting oneself professionally.
  • Promoting goodwill and confidence as a representative of LIFE Pittsburgh.
  • Proactively maintaining requirements for the position, including vehicle access, valid driver's license, auto insurance, and compliance with educational requirements.
