About The Position
Pleasant Hill Post Acute is seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehabilitation team. Requirements
- Current California Physical Therapist (PT) license
- Strong clinical and communication skills
- Commitment to resident-centered care
- Ability to work effectively within a multidisciplinary team
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services to improve mobility, strength, and independence
- Supervise PTAs and collaborate with the interdisciplinary care team
- Participate in care planning and discharge preparation
- Educate residents, families, and caregivers
- Maintain accurate and timely clinical documentation
