About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout the I-95 Corridor from Triangle to Springfield, Virginia. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful impact daily. As a Home Health Physical Therapist, you will provide one-on-one care to help patients regain strength, improve mobility, restore balance, and return to the life they love. You will have the freedom to create personalized treatment plans, build meaningful relationships, and see the direct impact of your care with every visit.

Requirements

Active Virginia Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Interpersonal Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky

Responsibilities

Complete physical therapy evaluations, reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
