About The Position

Requirements

  • Current, valid Physical Therapist license in the state
  • Graduate of an accredited Physical Therapy program (DPT preferred)
  • Exceptional verbal and written communication skills
  • Strong clinical assessment and critical thinking abilities
  • Ability to work independently with minimal supervision while managing time effectively
  • Excellent organizational skills and meticulous attention to detail
  • Current CPR/BLS certification required
  • Valid driver's license, reliable transportation, and current auto insurance
  • Ability to lift and transfer patients as needed

Nice To Haves

  • Previous home health physical therapy experience strongly preferred
  • Knowledge of OASIS documentation and Medicare home health regulations preferred

Responsibilities

  • Conduct comprehensive physical therapy evaluations and assessments of patients in their home environment
  • Develop and implement individualized patient care plans and treatment goals based on assessment findings
  • Provide evidence-based therapeutic interventions, manual therapy, and rehabilitation treatments in patients' homes
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare team members to coordinate optimal patient care
  • Monitor and document patient progress using electronic medical records (EMR) systems
  • Modify treatment plans as needed based on patient response and changing needs
  • Educate patients and family caregivers on therapeutic exercises, mobility techniques, and home safety recommendations
  • Ensure compliance with Medicare/Medicaid regulations, OASIS documentation, and professional practice standards
  • Participate in quality assurance and performance improvement initiatives
