About The Position
PIVOT Rehabilitation is hiring a Licensed Home Health Physical Therapist (PT) for a PRN home health physical therapist job providing in-home physical therapy services to adult and senior patients in Fulshear, Cinco Ranch, Brookshire Tx and surrounding areas. Requirements
- Bachelor’s, Master’s, or Doctorate in Physical Therapy from an APTA-accredited program
- Active Physical Therapist (PT) license in the state of practice or PT Licensure Compact eligibility
- Home health or rehab experience (1+ year required)
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Valid driver’s license, reliable transportation, and current auto insurance
Nice To Haves
- Knowledge of OASIS and home health regulations preferred
Responsibilities
- Perform home health physical therapy evaluations, including OASIS assessments
- Develop and update individualized Plans of Care (POC)
- Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
- Deliver interventions focused on mobility, balance, strength, gait training, and fall prevention
- Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments
- Educate patients and caregivers on safe transfers, home exercise programs, and use of durable medical equipment
- Complete accurate and timely documentation in the EMR
- Communicate with physicians and interdisciplinary care teams
- Supervise PTAs and home health aides, as applicable
- Follow all home health regulations, safety standards, and infection control policies
