About The Position
MercyOne Home Care is seeking a compassionate, mission-driven Physical Therapist to deliver exceptional physical therapy services in the comfort of patients’ homes. This role involves providing professional, evidence-based care, helping clients regain independence and improve their quality of life through personalized rehabilitation plans. The position is PRN, requiring 0-20 hours per week.
Requirements
- Degree from an APTA-accredited Physical Therapy program
- Active state PT license
- 1+ year of experience or completion of a 6–8 week preceptorship
- Valid driver’s license and reliable transportation
- Strong organizational and time-management skills
- Ability to lift up to 35 lbs and assist with mobility
- Frequent standing, walking, bending, and positioning
- Adaptability to changing schedules and environments
- Travel to client homes and Trinity Health locations required
Nice To Haves
- Home health experience preferred
Responsibilities
- Conduct comprehensive assessments and develop individualized treatment plans.
- Deliver therapeutic interventions (manual therapy, modalities, exercise programs).
- Educate clients, families, and caregivers on therapy techniques and home programs.
- Supervise PTAs and Home Health Aides (as permitted).
- Collaborate with interdisciplinary teams and participate in care conferences.
- Monitor progress and adjust treatment plans as needed.
- Maintain accurate, timely documentation.
