About The Position

Concierge Home Care is hiring a Home Health Physical Therapist (PT). This role offers more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity. The coverage area is Leesburg / Lake County, FL, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with additional earning potential and mileage reimbursement, ensuring consistent patient volume.

Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
