Home Care Physical Therapist (HH)
About The Position
Olympia Fields Hospital is seeking a dedicated and skilled Physical Therapist to join our Home Health Care team. This full-time position involves providing comprehensive physical therapy services to patients in their homes. The role requires performing thorough patient assessments, developing and implementing individualized treatment plans, and collaborating with physicians and other healthcare professionals. You will also be responsible for supervising Home Health Aides and actively contributing to the department's functions. Requirements
- Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.
- One year recent clinical experience.
- Current license as a Physical Therapist.
- Current BLS certification.
- Current driver’s license, car insurance, car registration, and reliable automobile.
Nice To Haves
- Home Care experience preferred.
Responsibilities
- Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals.
- Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods.
- Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers.
- Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.
- Demonstrates active participation in home care department functions.
