About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Lakeside and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule, allowing for flexibility and independence. You will have the opportunity to determine your own earnings based on the number of patients you treat. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, to allow you to focus on patient treatment. You will engage in one-on-one treatment, observe, record, and report patient responses, and determine the plan of care. Documentation is made easy with an intuitive Electronic Medical Record system. Requirements

• Current Physical Therapy license issued by the state board of Physical Therapy (must be current, active, and unrestricted; conditional, provisional, or restricted licenses will not be accepted; limited permits will not be accepted).
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
