About The Position
Haven Home Health is seeking an experienced and compassionate PRN Physical Therapist (PT) to join our growing team in the Orlando and surrounding areas. If you are passionate about helping patients regain independence in the comfort of their own homes, we would love to connect with you! As a Home Health PT, you will provide skilled physical therapy services to patients in their residences, focusing on restoring function, improving mobility, relieving pain, and preventing disability following illness or injury. This is a PRN (as-needed) position offering flexibility and competitive per-visit rates.
Requirements
- Current Physical Therapist license in Florida
- Current CPR certification
- Valid driver’s license and reliable transportation
Nice To Haves
- Minimum 1 year of home health experience preferred
- Strong clinical assessment and documentation skills
- Excellent communication and time-management skills
Responsibilities
- Perform initial evaluations and ongoing reassessments
- Develop and implement individualized care plans
- Provide therapeutic exercises, gait training, balance training, and fall prevention education
- Complete accurate and timely documentation
- Collaborate with physicians, nurses, and interdisciplinary team members
- Educate patients and caregivers on home safety and functional mobility
