About The Position

At COM, each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care. The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety. This role is different because Physical Therapists at COM: Lower patient volumes – no more seeing 3-4 patients an hour Form long-term relationships with participants rather than short-term rehab stints Collaborate with other therapists, providers, and disciplines Have work/life balance - no hours of documentation after work hours.

Requirements

  • Bachelor’s degree in physical therapy
  • Valid California Physical Therapy License, or proof of license eligibility
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter

Nice To Haves

  • Master’s or doctoral degree preferred
  • Two (2) years in clinical setting with frail or elderly population preferred
  • CPR and First Aid certification upon hire preferred
  • Ideally have outpatient, acute, inpatient, or skilled nursing experience

Responsibilities

  • Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
  • Actively participate in IDT meetings to develop participant care plans
  • Deliver and document interventions as agreed upon in participant care plans
  • Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
  • Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
  • Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment
