About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Washington, DC. As a Home Health Physical Therapist, you'll help patients regain strength, improve mobility, restore balance, and return to the activities that matter most—all while providing care in the comfort of their own homes. Requirements

Active Washington, DC Physical Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Interpersonal Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus

Responsibilities

Complete physical therapy evaluations, reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, balance, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
