About The Position

All About You Collaborative Health Care Services, LLC is growing and seeking experienced per diem Physical Therapists to join their team for the Stratford, Milford, and surrounding areas. As a Nurse owned and operated, transitional home care agency, they focus on transitioning patients from hospital/facility to home. They are a preferred provider for several local hospitals/facilities and Assisted Living Facilities. Requirements

Graduate from a physical therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
Currently licensed in the state(s) in which practicing.
Ability to exercise initiative and independent judgment.

Nice To Haves

Two (2) years homecare experience.

Responsibilities

Provides therapeutic services in the patient’s home setting.
Follows a specific written plan of care established in coordination with a physician.
Develops, implements, and evaluates the plan of care.
Provides coordination of the plan of care, while working collaboratively with other health professionals.
Identifies patient care needs, and makes recommendations for additional home health care services if needed.
Instructs, motivates, and assists patients/caregivers in learning and improving activities of the patient's daily living.
Demonstrates timely submission of paperwork and accurate computer documentation.
