About The Position
Pathways PACE is seeking a Physical Therapist for their new Greeley location to help shape the future of care for older adults in Northern Colorado. This role offers an opportunity to join a founding clinical team, build strong interdisciplinary relationships, and make a lasting impact on the lives of older adults by helping them remain independent and living safely in their own homes. The position offers meaningful patient relationships, a collaborative team, predictable weekday hours, and the chance to influence a new program.
Requirements
- Graduate of an accredited entry level program in Physical Therapy
- Active Colorado Physical Therapist license
- Current CPR certification
- Valid Colorado driver's license and proof of insurance
- One year working with elderly population, home care or geriatric experience

- Experience working with interdisciplinary teams
- An interest in preventative, community-based care
Responsibilities
- Build lasting relationships with participants and their families.
- Evaluate functional abilities and develop individualized treatment plans.
- Recommend home modifications and adaptive equipment that improve safety and independence.
- Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
- Help participants maintain meaningful daily activities and improve their quality of life.
- Play an important role in launching a new PACE program and helping establish best practices.
