About The Position
Physical Therapist (PT) – Home Health Visits Full-Time | Hudson County, NJ $10,000 Sign-On Bonus for Full-Time! BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our Hudson County Visits team. This position provides one-on-one home health visits for adult and geriatric clients throughout Hudson County, NJ , with an immediate opening in the southern Hudson County (Jersey City/Bayonne) area. Requirements
- Current New Jersey Physical Therapist (PT) license.
- Graduate of an accredited Physical Therapy program.
- Strong communication, organizational, and clinical assessment skills.
- Ability to work independently and effectively manage a flexible schedule.
- Reliable transportation and ability to travel throughout the assigned territory.
Nice To Haves
- Home health experience preferred, but not required.
- Experience with electronic medical records (EMR) preferred.
Responsibilities
- Perform skilled physical therapy visits throughout designated territories.
- Complete comprehensive physical therapy evaluations and ongoing reassessments.
- Assess mobility, strength, balance, gait, transfers, endurance, and overall functional status.
- Develop and implement individualized treatment plans using evidence-based interventions.
- Provide therapeutic exercise, gait training, balance training, manual therapy techniques, and patient education.
- Collaborate with nurses, physicians, and interdisciplinary team members to coordinate exceptional patient care.
- Educate patients and caregivers on exercises, safety strategies, fall prevention, and disease management.
- Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.
