About The Position
Innovate Rehab and Wellness is seeking a Licensed Physical Therapist (PT) to join their team. This position is located in a Skilled Nursing Facility in Jena, LA. The role focuses on providing one-on-one patient care, developing individualized treatment plans, and helping patients restore movement, function, and quality of life. Requirements
- Doctorate or Master’s degree in Physical Therapy (accredited program)
- Valid state PT license
- Strong communication, problem-solving, and patient-care skills
- CPR certification preferred
Nice To Haves
- Clinical experience preferred; new grads welcome
Responsibilities
- Perform comprehensive PT evaluations
- Develop personalized, evidence-based treatment plans
- Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
- Educate patients and caregivers on home programs and injury prevention
- Track and document progress; adjust plans as needed
- Collaborate with physicians, nurses, OTs, and other clinicians
- Stay current with PT best practices through continuing education
- Ensure compliance with all regulatory and ethical standards
