About The Position

All About Home Care is a Houston-based home health care agency serving the Houston and surrounding communities since 2004. We are looking for Physical Therapists (PTs) to serve patients on the East side (Northeast, Southeast, or Central East coverage) of the Greater Houston area. We are also seeking PRN PTs for The Woodlands and Conroe areas. Experience in home health is preferred. The agency uses Kinnser software for patient care documentation.

Requirements

Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
Currently licensed (licensed physical therapist) in the state of Texas.
Ability to exercise initiative and independent judgment.
Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Nice To Haves

Two years experience preferred.
Spanish speaking a plus

Responsibilities

Makes the initial assessment visit and generates OASIS documentation the same day of the visit
Understands and adheres to established policies and procedures.
Provides physician prescribed physical therapy.
Performs evaluation and treatment visits.
Improves or minimizes the residual physical disabilities of the patient.
Returns the individual to an optimal and productive level within the patient’s capabilities.
Participates with all other healthcare personnel in patient care planning.
Directs and supervises personnel as required.
Performs all skilled procedures as ordered by physician.
Consults with physicians regarding changes in treatment, reports to physicians regarding patient's progress, instructs patients and families/significant others in home programs and activities of daily living.
Participates in in-service programs and presents in-service programs as assigned.
Participates in performance improvement activities as assigned.
Attends all patient care conferences as scheduled.
Prepares clinical and progress notes daily.
Assists physicians with evaluating the level of function.
Helps develop the plan of care and revise as necessary.
Consults with family and agency personnel.
