About The Position

The physical therapist will assume autonomous responsibility for evaluating patient’s need for physical therapy. This will include the creation of treatment plans and the execution of those individual patient-based treatment programs in accordance with the State of Michigan Public Health Code Act 368 of 1978. The physical therapist will work cooperatively with other rehabilitation professionals including physical therapy assistants, rehabilitation technicians/aids, and athletic trainers in addition to the patient’s medical team to maintain standards of professional physical therapy practice and interdisciplinary teamwork. The physical therapist is responsible for the maintenance of their national certification, state licensure, and the necessary requirements for continuing education. The physical therapist is responsible to the service area manager and/or the designated site leader. This position is based in the House Rehab Services department in Muskegon, MI and does provide physical therapy services on an as-needed basis at Shelby Hospital in Shelby, MI.

Requirements

  • Must be a graduate of a Doctorate’s or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.
  • Bachelor's in Physical Therapy
  • FSBPT Certification.
  • State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
  • American Heart Association BLS or equivalent

Responsibilities

  • Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.
  • Evaluation and clinical diagnosis of patient conditions.
  • Establish treatment goals utilizing input from patient and their family as appropriate
  • Documentation remains in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.
  • Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan
  • Maintenance of national certification, state licensure, and necessary requirements for continuing education.
