HOW YOU'LL WORK: Provide comprehensive skilled services as ordered by the attending physician. Educate patients and their family members and ensure the safety of the patient. Accountable for completing accurate documentation and remaining compliant with all legal rules and regulations. QUALIFICATIONS & ATTRIBUTES: Licensure: Physical Therapist with a current license in the state of employment. Experience: Minimum of one year of experience as an PT in an acute setting is required; Home care experience is desired. Skills: Ability to effectively communicate and create positive impressions; knowledgeable on therapy best practices. Judgment: Ability to make appropriate physical therapy judgments and handle situations with the best possible solution. Work Ethic: Detail-oriented, observant, disciplined, and able to prioritize. Logistics: Valid driver’s license and auto insurance with your name as a listed driver. MAJOR AREAS OF RESPONSIBILITY: Patient Care & Treatment Provides skilled physical therapy in a patient’s home as ordered by the attending physician. Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means (exercise, massage, heat, water, etc.). Observes, records, and reports patient reactions, changes, and progress. Helps decrease re-hospitalizations by front loading visits for high risk patients and teaching preventative measures. Arranges for outpatient services that cannot be given in the patient’s home. Planning & Assessment Performs OASIS assessments to develop individualized plans of care. Completes initial evaluation and admission visit within 48-72 hours of referral. Evaluates and adjusts the POC based on patient response to treatment. Family & Team Communication Counsels and instructs patient/family on disease/injury management and equipment use (wheelchairs, walkers, etc.). Ensures patient/family know financial responsibility for equipment via DME department/MSW. Oversees and supervises therapy services provided by PTAs and nurse assistants. Communicates with schedulers, PTAs, and nurse aides regarding results and changes. Compliance & Administration Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Documents all aspects of treatment, assessments, and patient education. Participates in on-call requirements, case conferences, and mandatory meetings. Maintains a professional appearance and practices agency policies. ESSENTIAL FUNCTIONS Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documentation Timelines: Subsequent, discharge, eval/recert/resumption of care visits: Within 24 hours. Start of care visits: Within 48 hours. Establishes and follows plan of care as permitted within the scope of practice. HEALTH QUALIFICATIONS: Health Requirements: Must meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray. Safety: Must be free of conditions that restrict ability to perform job functions; risk for exposure to bloodborne pathogens exists. Physical Demands Constantly (66%-100%): Reading/Writing/Speaking English, Computer/PDA usage, Hearing/Seeing, Talking, Stretching/Reaching, Walking, Sitting, Bending, Standing.
