About The Position

Nightingale Home Healthcare has been proudly delivering compassionate, one-on-one home care for over 28 years. Our mission is to help patients regain independence and comfort in their own homes. We are seeking dedicated Physical Therapists (PTs) to join our growing team for part-time or per diem visits. This role offers flexibility, independence, and the chance to make a real impact.

Requirements

• Licensed Physical Therapist (PT)
• Current CPR certification
• Physical exam (within last 12 months)
• TB test (within last 12 months)
• Valid driver’s license
• Must attend orientation at our Taunton, MA office

Nice To Haves

• Minimum 1 year of PT experience preferred
• OASIS and WellSky (Kinser) experience preferred (training provided)

Responsibilities

• Evaluate patients’ physical conditions, limitations, and rehabilitation needs
• Develop and implement individualized treatment plans
• Provide therapy in accordance with the physician’s Plan of Care
• Supervise and coordinate care with PTAs and HHAs
• Maintain accurate documentation and progress notes
• Educate patients and caregivers on exercises and equipment use
• Assess home environments and recommend modifications
• Collaborate with nurses, social workers, and other healthcare providers
• Submit visit notes on time while maintaining HIPAA compliance
