Requirements

• Minimum of 9 months of experience as a Physical Therapist.
• Master's degree in Physical Therapy from a CAPTE-accredited physical therapy program.
• Current Physical Therapist license in the state of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service area.
• Strong communication, organization, and patient education skills.
• Passion for delivering compassionate, individualized care.

Nice To Haves

• Home health or community-based therapy experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations in accordance with physician orders and agency guidelines.
• Develop individualized treatment plans based on each patient's functional goals and clinical needs.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall function.
• Educate and counsel patients and caregivers on treatment plans, home exercise programs, and injury prevention.
• Communicate assessment findings and treatment recommendations with physicians and the interdisciplinary care team.
• Monitor patient progress, adjust treatment plans as needed, and establish measurable rehabilitation goals.
• Document evaluations, treatment sessions, progress notes, and care plans accurately and in a timely manner.
• Collaborate with team members to ensure coordinated, high-quality patient care.
• Maintain productivity standards while delivering exceptional clinical outcomes.
