About The Position

The Physical Therapist provides comprehensive and sympathetic care to patients in a Home Care and Hospice setting by assessing, planning, and implementing individualized therapy programs aimed at improving or maintaining functional mobility, alleviating pain, and enhancing quality of life. This role involves collaborating closely with interdisciplinary healthcare teams, patients, and their families to support optimal physical function and independence within the comfort of the patient's home. The Physical Therapist also educates caregivers on effective techniques and safety measures, ensures compliance with all regulatory and organizational standards, and contributes to the holistic well-being and dignity of patients during all stages of care, including end-of-life support.

Requirements

  • Degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist in New Hampshire.
  • Minimum of two years of experience as a physical therapist; community/home health experience preferred.
  • Strong verbal and written communication and organizational skills.
  • Current CPR certification.
  • Licensed driver with a reliable vehicle, insured in accordance with organizational requirements.

Responsibilities

  • Provide physical therapy services as outlined in the physician’s plan of care, including assessing and evaluating therapeutic/rehabilitative/functional status.
  • Evaluate the home environment and assess muscle strength, mobility, gait, and range of motion (ROM) with a focus on rehabilitation potential.
  • Direct physical therapy treatment and instruct patients and caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report patient reactions or changes in condition to the physician.
  • Initiate physical therapy programs and educate other personnel or family members on specific phases of therapy, participating in case conferences, as necessary.
  • Train patients in the use of prosthetic devices as needed.
  • Identify patient and family needs for other home health services and make referrals, as necessary.
  • Prepare and submit clinical and progress summaries based on goal attainment.
  • Participate in patient discharge planning.
  • Provide physical therapy consultation to families/caregivers when indicated and offer in-service education programs for nursing staff as needed.
  • Participate in the peer consultation process and supervise physical therapy assistants in accordance with organization policy and state regulations.
  • In cases where therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
