About The Position
Bridgeway Home Health is seeking a full-time Physical Therapist to join their clinical home-health field team. This role involves providing quality home health care to individuals in the state of Georgia, specifically in Forsyth County and surrounding areas. The mission of Bridgeway Home Health is to offer compassionate, home-based care, recognizing the valuable relationships with employees, providers, and the communities they serve.
Requirements
- Graduate of an accredited physical therapy educational program.
- Current Physical Therapist (PT) state license within the state of practice.
- Current CPR certification.
- Excellent organizational, time-management, written and verbal communication skills.
- Reliable transportation, valid driver’s license and current automobile insurance.
Nice To Haves
- Home Health / Oasis experience preferred.
Responsibilities
- Conducting one-on-one patient care in the home setting.
- Providing patient and family teaching in the home setting.
- Assessing patient needs in their homes.
- Developing personalized treatment plans.
- Implementing therapeutic interventions to optimize functional abilities.
