About The Position

M.D. Home Health, LLC is seeking a licensed Physical Therapist to join our home health team on a part-time basis. This position serves patients throughout Jupiter, Hobe Sound, Stuart, Port St. Lucie and surrounding communities within the assigned territory. This is an excellent opportunity for a PT who values flexible scheduling, professional independence and the ability to provide personalized, one-on-one care in patients’ homes.

Requirements

  • Active and unrestricted Florida Physical Therapist license
  • Graduate of an accredited physical therapy program
  • At least one year of clinical experience preferred
  • Home health experience preferred, but not required
  • Knowledge of Medicare home health guidelines and OASIS documentation is a plus
  • Current CPR or BLS certification
  • Valid driver’s license, active automobile insurance and reliable transportation
  • Ability to consistently travel throughout the Jupiter–Port St. Lucie territory
  • Strong clinical assessment, communication, time-management and documentation skills
  • Ability to work independently and manage a flexible, territory-based caseload
  • Successful completion of all required background screenings and employment requirements

Responsibilities

  • Conduct comprehensive in-home physical therapy evaluations and reassessments
  • Develop and update individualized plans of care based on patients’ functional needs and goals
  • Provide skilled interventions addressing mobility, strength, balance, gait, pain management and functional independence
  • Complete recertification and discharge visits when assigned
  • Monitor patient progress and adjust treatment plans as clinically appropriate
  • Educate patients and caregivers regarding home exercise programs, assistive devices, fall prevention and safety techniques
  • Communicate changes in patient condition to the physician and clinical supervisor
  • Collaborate with nurses, physical therapist assistants, occupational therapists and other members of the care team
  • Supervise physical therapist assistants when assigned and required
  • Complete accurate clinical documentation within established agency timeframes
  • Participate in care coordination and discharge planning
  • Follow physician orders, agency policies, HIPAA requirements and applicable state and federal regulations
