VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Rutland County. We are hiring for full-time and per diem positions working with adult and/or pediatric patients in the home health setting. POSITION SUMMARY As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in the home, facility, or educational environment. Caseloads may include adult and/or pediatric patients depending on experience and interest.

Requirements

  • Degree in Physical Therapy from an accredited program
  • Current Vermont PT license
  • Valid driver's license
  • Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.

Nice To Haves

  • Home health or prior clinical experience preferred
  • Ideal candidates are compassionate, organized, flexible, and comfortable working independently while remaining part of a collaborative care team.

Responsibilities

  • Completing PT and OASIS assessments as appropriate
  • Developing and implementing individualized plans of care
  • Providing therapeutic interventions and caregiver education
  • Monitoring patient progress and adjusting treatment plans as needed
  • Collaborating with interdisciplinary team members
  • Supervising PTAs, students, and home care aides when applicable
  • Maintaining timely and accurate clinical documentation
