About The Position

Responsible for providing direct and consultative physical therapy services to Participants; develops, implements, monitors and documents client progress; coordinates procurement, maintenance and use of equipment.

Requirements

  • Masters degree in Physical Therapy and valid, current license to practice in Pennsylvania required.
  • PA Act 33 and PA Act 169 clearances required.
  • Must have excellent written and verbal communication skills, professional interpersonal, effective time management, Complex problem-solving, Instructing and teaching, and basic computer skills.
  • Ability to work in fast-paced environment.
  • Strong sensory skills, such as visual acuity, good hearing and dexterity.
  • Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain (with appropriate training), bend, squat, reach, push up and stand for several hours at a time.

Nice To Haves

  • Willingness to become a Certified Brain Injury Specialist within one year of starting this position.

Responsibilities

  • Performs Participant evaluation, treatment, and supervision of treatment as prescribed by Participants’ Primary Care Providers and in accordance with PT Standards of Practice, and Medicare requirements and with sensitivity for individual and cultural differences.
  • Maintains 70% or greater productivity
  • Prepares and documents accurate records of Participants’ treatment, tests, reactions and evaluation of progress as required by Woods Services’ policies and Medicare and other funders’ requirements.
  • Prepares and submits accurate and timely records of Participants' treatment in the electronic health record, tests reactions and evaluation of progress with each treatment session and as required for programming and/or insurance billing.
  • Treatment notes: to be completed in Woods Services Electronic Health Record System at the time of service delivery or within 24 hours.
  • Evaluations and assessments: submitted in Woods Services Electronic Health Record System at the time of completion or within 24 hours.
  • Monthly /Quarterly, semi-annual reporting: to be submitted to Woods Services Electronic Health Record System by established due dates.
  • Participates in Rehabilitation Service Plan teams for assigned Participants; develops habilitation and service goals and objectives, monitors/report same and modifies treatment plans when needed.
  • Acts as a resource person, providing staff training as needed to meet the needs of Participants.
  • Directs staff in care and use of wheelchairs, braces, ambulatory and orthotic
  • Attends ongoing continuing education and trainings assigned by Woods that ensures compliance with training regulations and maintaining Physical Therapy license.
  • Supervises assistants, aides, students and volunteers assigned to the PT Area, with sensitivity to individual and cultural differences and in accordance with the Standards of Practice.
  • Responsible for documentation and maintenance of appropriate records including initial evaluations, annual evaluations, RSP progress notes, discharge summaries, and other reporting documentation required by funding sources.
  • Attends and participates in various meetings and training sessions as an active part of the health care team and Beechwood community.
  • Addresses repairs regarding Participant equipment (wheelchairs, ambulation aids, braces, etc.) in accordance with case management, physician/PCP, adaptive equipment specialists, and/or vendors.
  • Willingness to become a Certified Brain Injury Specialist within one year of starting this position.
  • Performs other duties as assigned
