About The Position
Ultimate Therapy is seeking a motivated Physical Therapist (PT) to join our rehabilitation team. This full-time opportunity is ideal for a therapist who enjoys helping residents restore mobility, build strength, and achieve greater independence in a collaborative skilled nursing and post-acute care environment.
Requirements
- Active Connecticut Physical Therapist license required
- Graduate of an accredited Physical Therapy program
- Strong communication and clinical decision-making skills
Nice To Haves
- Skilled nursing or post-acute experience preferred but not required
- New graduates are encouraged to apply
Responsibilities
- Perform comprehensive physical therapy evaluations
- Develop individualized treatment plans based on resident needs and functional goals
- Provide skilled therapy focused on strength, balance, gait, mobility, transfers, and endurance
- Monitor resident progress and modify treatment plans as appropriate
- Educate residents, families, and caregivers on exercises and safe mobility techniques
- Collaborate with PTAs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team
- Complete accurate and timely clinical documentation
