About The Position

Aspire Home Healthcare is seeking a skilled Physical Therapist (PT) to join their team and help patients regain independence through one-on-one home health care. This role involves evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team. Requirements

• Active Physical Therapist (PT) license in OH
• Minimum 1 year of PT experience preferred
• Current TB test (within 12 months) and CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures
• Evaluate patient condition and establish measurable treatment goals
• Develop and implement individualized treatment plans
• Supervise and coordinate care with PTAs and HHAs
• Document patient progress, updates, and treatment notes accurately and on time
• Educate patients and families on exercises, mobility, and use of adaptive equipment
• Evaluate home environments and recommend modifications to improve safety and independence
• Ensure all services meet HIPAA standards for confidentiality
• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
