About The Position
Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for hospice patients in a home setting. The role focuses on helping older adults maintain their independence and quality of life through compassionate, evidence-based care. Therapists evaluate and treat residents in an assisted living community and/or home setting. This is a PRN, as-needed role; therapists choose which assignments to accept and maintain control of their schedules. The position offers clinical autonomy and a supportive team environment.
Requirements
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Active Florida Physical Therapist (PT) license.
Responsibilities
- Evaluate and treat residents in an assisted living community and/or home setting.
- Develop and implement individualized treatment plans.
- Collaborate with physicians, families, and the interdisciplinary care team.
- Maintain accurate, timely documentation in the EMR.
