Requirements

Current and active California Physical Therapist (PT) License
Strong communication and interpersonal skills
Commitment to providing compassionate, resident-centered care

Nice To Haves

Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, Long-Term Care, Rehabilitation, or Geriatric Care
New graduates are encouraged to apply

Responsibilities

Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence
Implement evidence-based rehabilitation interventions
Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams
Educate residents, families, and caregivers on therapy goals, treatment plans, and safety techniques
Document patient progress accurately and timely in accordance with regulatory and facility standards
Assist residents in achieving their highest practical level of function and quality of life
