About The Position

Innovate Rehab and Wellness is a therapist- and veteran-owned skilled health care provider with over 25 years of experience. We are dedicated to industry-leading, patient-centered care and offer a supportive, collaborative clinical team. This full-time or PRN position is located in a Skilled Nursing Facility in St Louis, MO, with a flexible Monday-Friday schedule. The role involves a split with another local facility and offers a pay rate of $42.00 - $70.00 based on experience, location, and availability. We focus on one-on-one patient care, realistic scheduling for work-life balance, and strong emphasis on clinical outcomes and patient satisfaction, with opportunities for professional growth and development.

Requirements

Doctorate or Master’s degree in Physical Therapy (accredited program)
Valid state PT license
Strong communication, problem-solving, and patient-care skills
CPR certification preferred

Nice To Haves

Clinical experience preferred; new grads welcome

Responsibilities

Perform comprehensive PT evaluations
Develop personalized, evidence-based treatment plans
Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
Educate patients and caregivers on home programs and injury prevention
Track and document progress; adjust plans as needed
Collaborate with physicians, nurses, OTs, and other clinicians
Stay current with PT best practices through continuing education
Ensure compliance with all regulatory and ethical standards
