About The Position

Flourish Health Clinic, part of Mental Health Cooperative (MHC), is seeking a Licensed Physical Therapist to deliver trauma-informed physical therapy services. MHC is dedicated to supporting individuals with severe mental illness and children with severe emotional disorders across Middle and East Tennessee. We are committed to providing comprehensive, multidisciplinary care in a collaborative and innovative environment. This role is crucial in helping patients manage pain, improve mobility, and regain functional independence.

Requirements

  • Licensed physical therapist.
  • Ability to deliver trauma-informed physical therapy services.
  • Proficiency in performing musculoskeletal and neuromuscular assessments.
  • Skill in developing and implementing individualized treatment plans.
  • Experience with hands-on therapy techniques such as manual therapy, stretching, and therapeutic exercises.
  • Ability to instruct patients in home exercise programs.
  • Experience addressing acute and chronic conditions.
  • Competence in fitting and educating patients on proper use of assistive devices.
  • Experience documenting patient interactions and progress within an electronic health record system.
  • Ability to collaborate with physicians, nurses, case managers, and social workers.
  • Skill in educating patients on body mechanics, injury prevention, and strategies for maintaining physical health.

  • Experience working with patients with limited access to follow-up care or rehabilitation facilities.
  • Experience working with individuals challenged with serious mental illness and poverty.

Responsibilities

  • Perform musculoskeletal and neuromuscular assessments.
  • Develop and implement individualized treatment plans tailored to patients with limited access to follow-up care or rehabilitation facilities.
  • Provide hands-on therapy techniques such as manual therapy, stretching, and therapeutic exercises.
  • Instruct patients in home exercise programs using easily accessible equipment or bodyweight techniques.
  • Address acute and chronic conditions such as back pain, joint dysfunction, gait instability, and post-operative rehabilitation.
  • Fit and educate patients on proper use of assistive devices (e.g., canes, walkers, braces).
  • Document patient interactions and progress clearly and securely, in coordination with the clinic’s electronic health record system.
  • Collaborate with physicians, nurses, case managers, and social workers to support whole-person care and identify referral options when needed.
  • Educate patients on body mechanics, injury prevention, and strategies for maintaining physical health despite environmental challenges.
