About The Position
AHN Healthcare@Home is hiring a Physical Therapist for a home health position serving patients in their homes in the Waynesburg, Prosperity, and Amity, Pennsylvania area. The role offers autonomy, meaningful patient relationships, and career growth, with support from a dedicated field education team and an interdisciplinary care team. The position is eligible for a $10,000 first-year success bonus tied to key achievements, with additional bonus opportunities as employees continue to grow with the organization. The Physical Therapist delivers skilled, patient-centered care, evaluates patients and their functional mobility, and develops individualized therapy plans to improve strength, balance, and independence. The therapist educates patients and caregivers, documents care in the EMR, and travels independently to patient homes within the assigned territory. Approximately 20% of the workday is spent driving to and from patient locations.
Requirements
- Active Physical Therapist license in the state where the employee works.
- One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
- Valid driver’s license.
- Ability to spend approximately 20% of the workday driving to and from patient locations.
Responsibilities
- Deliver skilled, patient-centered care in patients’ homes in collaboration with an interdisciplinary team.
- Perform comprehensive patient evaluations and functional mobility assessments.
- Develop individualized therapy plans to improve patients’ strength, balance, and independence.
- Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.
- Solve problems in real time and maintain thorough, compliant EMR documentation.
- Travel to patient homes within the assigned territory.
