Requirements
- Active Minnesota Physical Therapist (PT) license.
- Strong clinical reasoning and independent decision-making skills.
- Passion for delivering patient-centered care.
- Excellent communication and relationship-building abilities.
- Valid driver's license and reliable transportation.
Responsibilities
- Evaluate patients in their homes and develop individualized treatment plans.
- Improve strength, balance, mobility, endurance, and functional independence.
- Help patients safely navigate their home environments and prevent future injuries.
- Educate patients and caregivers to maximize long-term success.
- Collaborate with physicians and the interdisciplinary care team to achieve the best outcomes.
- Maintain timely and accurate clinical documentation.
