Requirements

  • Graduate of an accredited Physical Therapy program.
  • Current Ohio Physical Therapist (PT) license in good standing.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Strong evaluation, clinical reasoning, documentation, communication, and organizational skills.
  • Passion for delivering exceptional care and helping patients achieve meaningful outcomes.

Nice To Haves

  • Home health and OASIS experience preferred but not required.

Responsibilities

  • Perform comprehensive physical therapy evaluations, Start of Care (SOC), Resumption of Care (ROC), Recertification, Reevaluation, and Discharge assessments.
  • Complete accurate and timely OASIS documentation in accordance with Medicare and agency standards.
  • Develop individualized Plans of Care with measurable functional goals and evidence-based interventions.
  • Provide skilled therapeutic exercise, gait training, balance activities, transfer training, fall prevention strategies, and functional mobility interventions.
  • Monitor patient progress and modify treatment plans based on clinical findings and patient goals.
  • Supervise Physical Therapist Assistants (PTAs) and ensure quality, compliant delivery of care.
  • Educate patients and caregivers on home exercise programs, safe mobility techniques, assistive devices, and strategies to maximize independence.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate exceptional patient care.
  • Complete timely, accurate clinical documentation that reflects skilled need, patient progress, and medical necessity.
  • Demonstrate accountability and ownership in achieving outstanding patient outcomes while maintaining compliance with all agency, state, and federal regulations.
