About The Position

This role plans, organizes, and delivers physical therapy programs to help clients/patients/residents recover and improve their mobility, relieve pain, increase strength, and prevent deformities. The Physical Therapist conducts examinations, evaluations, and interventions for clients/patients/residents affected by injury or disease, utilizing appropriate tools and techniques while adhering to safeguards. They establish and modify therapeutic plans, educate patients and their families on treatment plans and home exercises, and apply age-related treatments and evaluations. The position requires demonstrating and incorporating current research into daily practice, potentially treating individuals of all ages with various diagnoses and disorders. Documentation must meet departmental guidelines and regulatory requirements, and treatment should align with the accepted clinical practice guidelines of professional organizations. Flexibility with work schedules and assignments is expected, as is participation in activities promoting the department, Good Samaritan Society, and Sanford Health. Collaboration with external individuals such as providers, case managers, and third-party payers is also part of the role. There is a possibility of supervising assistants, aides, and students, and providing ongoing education to peers in areas of expertise. Depending on facility needs, the role may focus on post-acute, long-term care, or home health care environments.

Requirements

  • Appropriate education level required in accordance with state licensure.
  • Must be licensed in the state(s) of practice as a Physical Therapist.
  • When applicable, may require valid driver's license.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

Responsibilities

  • Plans, organizes, and delivers physical therapy programs.
  • Conducts examinations, evaluations, and interventions for clients/patients/residents.
  • Utilizes tools and techniques to administer physical therapy interventions following safeguards.
  • Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary.
  • Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises.
  • Applies appropriate age-related treatments and evaluations.
  • Demonstrates and incorporates knowledge of current research into daily treatment.
  • May treat individuals of all ages with varying diagnosis and disorders.
  • Demonstrates documentation in accordance with departmental guidelines and meets all regulatory requirements.
  • Demonstrates level of treatment associated with their specialty.
  • Follows and adheres to accepted clinical practice guidelines of professional organization.
  • Exhibits flexibility with acceptance of variable work schedules/assignments.
  • Participates in activities to promote the department, Good Samaritan Society and Sanford Health.
  • Interacts with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc.
  • May be asked to supervise assistants, aides, and students.
  • Provides ongoing education and training to peers in the area(s) of expertise.
  • Performs other related duties as requested by manager.
  • Based on facility needs and assigned locations, may focus on those receiving intervention in the post-acute, long term care and home health care environments.
