About The Position
Nursing Vault Plus Staffing is seeking a dedicated and professional Physical Therapist for a 14-week contract assignment at Chapel Manor in Philadelphia. This role is ideal for a clinician who is passionate about geriatric rehabilitation and helping patients regain their independence in a skilled nursing environment.
Requirements
- Graduate of an accredited Physical Therapy program (Master’s or Doctorate).
- Active Pennsylvania Physical Therapist license.
- Current CPR/BLS certification.
- Strong clinical reasoning and proficiency in geriatric rehabilitation.
- Excellent communication skills to motivate patients and collaborate with staff.
Responsibilities
- Conduct thorough initial evaluations of patient mobility, strength, and functional limitations.
- Develop and implement individualized 14-week treatment plans with measurable goals.
- Administer manual therapy, therapeutic exercises, and gait training to improve patient outcomes.
- Work closely with the Chapel Manor interdisciplinary team (OTs, SLPs, and Nursing) to coordinate comprehensive care.
- Maintain timely and accurate clinical records in compliance with facility and state standards.
- Instruct patients and families on safe mobility techniques and home exercise programs.
