About The Position

Christian Health is seeking Per Diem Physical Therapists to join their Outpatient Rehab team. This role involves managing patient care, developing treatment plans, and collaborating with an interdisciplinary team. The Outpatient Rehab offers expert physical, occupational, and speech therapy services with a focus on personalized treatment plans and innovative approaches to achieve optimal patient outcomes. The Physical Therapist will be responsible for the quality of patient care outcomes, efficient resource management, and supervising assistants and aides according to state and federal guidelines. Requirements

  • Graduate of an accredited college or university with a bachelor’s degree in physical therapy.
  • Must possess a current, unencumbered license to practice as a licensed Physical Therapist in the State of New Jersey.
  • Must be able to lift a minimum of 50 pounds without assistance.
  • Demonstrates compliance with the CH ASPIRE Standards of Performance.

Nice To Haves

  • CPR certification preferred.

Responsibilities

  • Manage the care of the patient, managing quality of all related outcomes.
  • Participate as an integral part of the interdisciplinary team.
  • Manage resources to meet the needs of the patient balancing outcomes to provide effective and efficient treatment.
  • Supervise clinically all assistants and aides of patients to which the Therapist is directing or overseeing the delivery of services according to an established plan of care.
  • Provide and direct patient rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
  • Evaluate patient's needs and establish effective and efficient treatment plans for the patient assigned.
  • Manage and coordinate the supervision of treatment plans with all staff and other disciplines as indicated.
  • Participate in the screening of all patients according to directed schedules and practices.
  • Provide feedback to the designated supervisor on all clinical and rehabilitation issues.
  • Participate in all center directed processes and services.
  • Show effective management of treatment and non-treatment related activities.
  • Participate in in-service training programs.
  • May travel or float to other CH communities to provide services.
  • Complete all necessary paperwork for clinical and billing documentation.
  • Treat patients in accordance with assignment and plan of care.
  • Participate in center and patient specific meetings as needed.
  • Assist in the maintenance and cleanliness of the department and equipment.
  • Provide supervision to assistants and aides/techs.
  • Complete weekly reports and patient summaries.
  • Participate in weekly meetings.
  • Update and renew patient orders and plan of care as indicated.
  • Participate in monthly meetings.
  • Assure complete monthly billing.
  • Renew patient orders and update plan of care as indicated.
  • Participate in quarterly quality improvement initiatives as Directed by DOR.
  • Participate in quarterly reviews of long-term residents as directed by DOR.
  • Participate in annual review.
  • Assist DOR in annual review of other staff as appropriate.
  • Perform other duties as required.
