About The Position
PurposeCare is hiring a Physical Therapist to provide home health care to patients in their homes. The role involves skilled visits that address patients’ needs in their home environments, supporting recovery and progress in daily life. Clinicians have autonomy in clinical decision-making, manageable caseloads, flexible scheduling, and opportunities to build one-on-one relationships with patients. PurposeCare provides ongoing training and professional development, mentoring, referral programs, and opportunities to apply for positions at other locations.
Requirements
- Current Physical Therapist license in the state where applying.
- Ability to perform physical tasks that may include lifting up to 50 pounds, bending, and standing.
- Current driver’s license and vehicle insurance, and/or access to dependable transportation.
- Ability to successfully complete a background check.
Nice To Haves
- One year of physical therapist experience is preferred.
- Home health experience is preferred.
- OASIS experience is preferred.
Responsibilities

- Make clinical decisions independently using professional expertise.
