Requirements

  • Graduate of an accredited Physical Therapy program required.
  • Current state Physical Therapist (PT) license in good standing required.
  • Minimum of one (1) year of experience in physical therapy required
  • Current CPR certification required
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to pass background screening and all compliance requirements

Responsibilities

  • Perform initial and ongoing patient evaluations to assess mobility, strength, balance, endurance, and functional limitations
  • Complete comprehensive assessments and participate in OASIS-related coordination as applicable
  • Develop, implement, and update individualized physical therapy treatment plans
  • Ensure physician orders are appropriate and aligned with the patient’s therapy needs
  • Provide skilled physical therapy interventions in accordance with physician orders and established care plans
  • Guide patients through therapeutic exercises, gait training, transfer training, balance activities, and mobility restoration
  • Instruct patients in the safe use of assistive devices including walkers, canes, wheelchairs, and other mobility equipment
  • Support fall prevention, pain management, and restoration of safe functional movement in the home setting
  • Teach and counsel patients and caregivers regarding therapy goals, home safety, fall prevention, mobility strategies, and self-care techniques
  • Evaluate the home environment and identify safety modifications or community resources needed to support care
  • Coordinate outpatient services when procedures cannot be safely performed in the home
  • Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries
  • Evaluate effectiveness of therapy interventions and update the plan of care as needed
  • Maintain compliance with Medicare requirements, state regulations, and agency policies
  • Communicate regularly with physicians, nurses, and interdisciplinary team members regarding patient progress and needs
  • Supervise the Physical Therapist Assistant (PTA) in accordance with agency policy and regulatory requirements
  • Supervise the Home Health Aide when Physical Therapy is the only skilled discipline involved in patient care
  • Participate in care conferences, discharge planning, staff meetings, and required in-services
