About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Hesperia and the surrounding areas. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, allowing clinicians to focus on patient treatment. The role involves one-on-one treatment, determining the plan of care, and utilizing an intuitive Electronic Medical Record system for easy documentation, often completed during the visit. The company aims to connect clinicians with homebound patients who might otherwise go without care, contracting with numerous home health agencies across multiple states.

Requirements

• Ability and enthusiasm in working with all ages
• Current Physical Therapy license issued by the state board of Physical Therapy
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapist program
• Reliable personal transportation
• Smartphone

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using an Electronic Medical Record system.
