About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care across the South Shore of Nassau County. This role offers flexibility and autonomy, allowing you to choose your designated area and see patients any 7 days of the week. We welcome all levels of experience, including new graduates, and will provide training and mentorship. Join a growing organization with a supportive and innovative team.

Requirements

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible
• Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
