About The Position
Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Remington, Opal, and surrounding Fauquier County communities. As a Home Health Physical Therapist, you will help patients regain strength, improve mobility, reduce pain, and restore confidence in the comfort of their homes. You will have the freedom to focus on quality care, develop personalized treatment plans, and build meaningful relationships with patients, providing one-on-one therapy that produces real results.
Requirements
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Interpersonal Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge visits
- Develop individualized plans of care based on patient goals and clinical needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, balance, and home safety
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
