Requirements
- Must possess a current, valid Pennsylvania PT license and have current CPR certification
- Valid driver's license and ability to travel to case assignments
- Must be comfortable working in home setting
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal and communication skills
Responsibilities
- Evaluate patients and develop plans of care to achieve desired outcomes
- Provide therapeutic treatment modalities and implement home therapy / exercise programs
- Provide instruction to patients to achieve patient goals
- Maintain communication with ordering physicians and clinical team to promote progress toward goals or seek intervention when needed
- Participate in quality improvement activities as assigned
- Timely and accurately document (EMR) all patient assessments, interactions, etc.
