About The Position

Brookestone Home Health is seeking a compassionate and skilled Physical Therapist (PT) to join our team on a Part-Time or PRN basis. This position provides the opportunity to deliver one-on-one care to patients in their homes throughout the Omaha metro area, helping them regain independence, improve mobility, and achieve their highest level of function. We are looking for a clinician who values patient-centered care, flexibility, and the ability to make a meaningful impact in the lives of patients and their families.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Nebraska Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Ability to travel throughout the Omaha service area.

Nice To Haves

• Home health experience.
• Experience working with geriatric and rehabilitation populations.
• Knowledge of Medicare and home health documentation requirements.

Responsibilities

• Conduct physical therapy evaluations and reassessments.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise, gait training, balance training, transfer training, and fall prevention education.
• Assess the need for assistive devices and durable medical equipment.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Monitor and document patient progress toward established goals.
• Collaborate with physicians, nurses, and other healthcare professionals regarding patient care.
• Complete clinical documentation accurately and in a timely manner.
• Maintain compliance with agency policies, professional standards, and regulatory requirements.
