About The Position

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in New Iberia, Louisiana, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians may also incorporate telehealth and other virtual care into their schedules. The role supports patients’ mobility, independence, wellness, longevity, and quality of life. Physical Therapists perform evaluations, develop individualized plans of care, provide evidence-based treatment, document in FOX’s EMR system, and collaborate with an interdisciplinary team. The position offers clinician-driven scheduling and opportunities to build lasting patient relationships. FOX lists full-time, part-time, and PRN / FOX Flex employment pathways. Compensation varies according to experience and employment type. Benefits are available for eligible roles. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians.

Requirements

Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
